Preferred Provider Organization (PPO) offers in and out-of-network reimbursement. Your insurance policy may cover a portion or all of your visit. I can provide you with paperwork to submit to your insurance company for reimbursement.
Do not forget about Health Savings accounts or Flex spending accounts that you can use to cover your visits. I can provide you with receipts as needed.

Late Cancelations and Missed Appointments
Late Cancellations and missed appointments are not billable to your insurance company. When you make an appointment with me, you purchase that time dedicated to you. You are responsible for that time unless you cancel 48 hours before your appointment. The flat rate is $200.00 for intake appointments, $150.00 for hour appointments or $75.00 for 30-minute appointments.
